Overview
Some forms required to do business with the district clerk are available online below. Most can be printed, filled out, and either mailed or faxed to the district clerk unless specified otherwise.
Order Requiring Completion of Parenting Class
Affidavit in Support of Court Appointed Attorney
- Affidavit in Support of Court Appointed Attorney: To be considered for court appointed counsel, every question on the attached form must be answered. Click here for instructions for completing Affidavit in Support of Court Appointed Attorney
Issuance of Process Instructions
- Issuance of Process Instructions: This form is required before we will issue any process including but not limited to: Citation, Citation by Publication, Citation by Posting, Writ of Sequestration, Writ of Garnishment, Writ of Attachment, Subpoena -Civil and Criminal, Bill of Cost.
- Issuance of Process Instructions for Writ of Execution: This form is required before we will issue any Writ of Execution or Abstract of Judgment.
Public Information Request
- Request Form: This form is required when requesting public information from the District Clerk, including civil and criminal searches and copy requests.
Setting Request (Civil / Criminal)
- Setting Request Form: This form should be used when requesting a hearing with the county courts at law and/or the District Courts of Brazos County.
Occupational Driver's License
- Occupational Driver's License Petition: This form should be used when requesting an occupational license (for a revoked or suspended license).

Information on Suit Affecting the Family Relationship
- Information Required by Texas Family Code: This form is required in all divorce cases and suits affecting the parent child relationship.
- Information on Suit Affecting the Family Relationship (BVS Form): All divorces / annulments (with or without children) and all suits affecting the parent-child relationship must be reported through the clerk of the court to The Texas Vital Statistics Unit.
Civil Case Information Sheet
As of September 1, 2010, a Civil Case Information Sheet must be completed when filing a civil suit.

Mandatory Discovery Form
- Mandatory Discovery Form: This form is available for convenience of the State and Defense, to ensure compliance with Texas Code of Criminal Procedure Article 39.14 and to avoid any trial delays due to discovery or witness conflicts.
